There is no cost to you for any of the services. All the programs and services offered by the HFHT are funded by the Ontario Ministry of Health and Long-Term Care.
To see your family physician simply call the Hanover Medical Associates to book an appointment. To see one of the HFHTs allied health professionals, talk to your family physician about a referral. Once the referral has been made, you will be contacted by a member of the HFHT to arrange an appointment. You can also self-refer by calling the HFHT directly and booking an appointment. You must be a registered patient of the Hanover Medical Associates in order to utilize the services at the HFHT.
