The HFHT allows patients to access specialized health care services in a timely manner. Our ‘no barriers’ approach to healthcare means support is always available if you need it. Our services focus primarily on disease prevention, health promotion and disease management.
Please contact the Hanover Family Health Team office at 519-506-4348 as soon as possible and preferably at least 24 hours prior to your scheduled appointment to cancel.

A call to Telehealth Ontario will give you confidential advice about any health-related concerns such as:

Nurse Practitioners provide care to patients of all ages with a focus on preventing disease, promoting good health and treating illness. They are part of your health care team. The Nurse Practitioner plays an important role in helping you to maintain a healthy, active lifestyle, and to understand and/or treat your illness. They work with our community to promote good health and prevent disease. As a member of your health care team, our Nurse Practitioner offers a variety of health care services including the ability to:
- diagnose and treat illness and/or injuries
- perform physical check-ups
- order and interpret diagnostic tests
- write prescriptions
- provide counselling and education
- provide supportive care through illness
- provide treatments and/or procedures
- make referrals to family physicians, specialists and other health professionals
