- Hanover Family Health Team is seeking a Registered Kinesiologist
Part-time
The Hanover Family Health Team is seeking a collaborative Registered Kinesiologist to join our primary care practice on part-time basis (2 days for a total of 15 hours per week). This position provides specialized exercise education and treatment to adults living with chronic diseases. This role also works alongside our interdisciplinary team in support of our Boosting Balance, Stride Alive and Cardiac Rehab programs. The Registered Kinesiologist develops, implements, monitors and evaluates services aimed at reducing the onset of chronic diseases and reducing complications among patients who already have a chronic disease diagnosis.
Requirements
Bachelor degree in Kinesiology from an accredited university and registered with the College of Kinesiologists of Ontario.
Two or more years’ experience in a community and/or primary health care setting preferred.
Ability to provide patient assessments in areas within scope of practice and implement various goal-oriented intervention/exercise plans for chronic conditions such as diabetes, COPD, CVD, weight impacting health, arthritis, chronic pain.
Confidence with leading group exercise and education programs.
Ability to work independently within scope of practice and also collaborate in an inter-disciplinary team to support patient needs.
Experience working with Electronic Medical Records
Additional clinical certification or experience in a specialty area is an asset, e.g. Clinical Exercise Physiologist
Reliable transportation to work.
Up-to-date vaccinations (or willing to get up-to-date).

2. Hanover Family Health Team is seeking a Social Worker/Mental Health Counsellor
Full-time (part-time considered)
The Hanover Family Health Team is seeking a collaborative Social Worker/Mental Health Counsellor to join our primary care practice on either a permanent full-time (37.5 hours per week) or part-time basis (up to 22.5 hours per week). This clinician will assist with referrals for mental health services and provide brief/short-term therapeutic counselling services. This role also supports our Memory Clinic program and works alongside our Memory Clinic interdisciplinary team. This role also connects with patients to review broader health and social needs and provide information and referral to support patient and caregiver system navigation.
Requirements
Masters degree in social work, counselling, psychology or equivalent in a clinically relevant field from an accredited university with a minimum 5 years mental health related work experience.
Advanced generalist counselling skills to be used to provide brief /short-term counselling to assist in the treatment of an array of adult mental health and psycho-social problems including, but not limited to: depression, anxiety, adjustment problems, bereavement and loss, stress, work problems, domestic violence, trauma, chronic pain, aging issues, suicidal ideation and crisis planning, eating disorders.
Ability to work independently within scope of practice and also collaborate on an inter-disciplinary team to support patient needs.
Registered with one of the following regulatory colleges: Ontario College of Social Workers and Social Services Workers; College of Occupational Therapists of Ontario; College of Registered Nurses of Ontario; College of Registered Psychotherapists of Ontario.
Additional clinical certification or experience in a specialty area is an asset, e.g. trauma, children and youth.
Reliable transportation to work.
Up-to-date vaccinations (or willing to get up-to-date).
